We are in a doctor's office sending these to our patients and the feed back from patients is terrible. At least 3 times a week, someone complains that the statement seperated at the perforation before they even got it out of the envelope and that makes it very difficult for patients when there are 4 pages all torn off at the top. The paper we used to get from MAP never did this but it has been a problem for about 2 years now. We are now switching to regular colored paper which is not as convenient but will not tear as easily.

We send our patient statements on these forms. The layout is perfect for us. The preforated return portion makes it easy for our patients to pay their balances. The only thing we would ask for is a greater selection of colors.
