    This is a very useful product to place in patients chart to prove date of service and insurance they said they had on that date. Do wish the "sign next line" would be much darker. It's hard for patients to see!

                It would be helpful if the "USE NEXT LINE" was printed on the top sign in page instead of under because patients write over each others names. They can't seem to "see" the USE NEXT LINE tag.
                Also, a PLEASE PRINT NAME area would be helpful. It's hard to read signatures!
