This self-sealing envelope is used to mail CMS-1500 forms to insurance companies. I've had a few returned to me due after mailing to find the perforated portion of the envelope opened during the mailing process. Anyone in the postal system could have looked at the PHI of my patient. I have to seal then tape to insure proper closure. Why the perforated portion of the seal???
